Please refer to the list below for insurances for which we are a provider, or call our office for our most up-to-date listing for specific plans. If we do not take your insurance, we will be glad to provide you with an itemized receipt so that you may submit an out-of-network claim to your insurance.

Vision Plans VS. Medical Insurance

We request all patients to bring both their medical and vision insurance coverage information. This is because eye exams fall into two main categories:

Medical: Some patients are not aware that eye exams can be medical in nature, which are required by insurance rules to be filed with your medical insurance. Our medical exam and diagnostic testing fees associated with a medical eye examination varies, and these fees range anywhere from $45.00 on up. Some such situations requiring a medical eye examination include:

Significant medical history (diabetes, high blood pressure, thyroid disease or autoimmune disease)

Pre-existing eye conditions (cataracts, glaucoma, retinal disease, etc.)

High risk medications and the monitoring of potential ocular side effects (Plaquenil, Tamoxifen, Amiodarone, Accutane, etc.)

Medical complaints (pain, redness, flashes, floaters, headaches, dryness, redness, itching, pain, irritation, swelling, blurry vision due to medical eye issues, etc.)

Vision: The analysis, diagnosis, treatment or monitoring of medical eye conditions are specifically not covered by certain vision plans. Vision plans provide coverage for standard wellness “routine” eye examinations. Our standard wellness vision exam associated with this type of visit is $95.00, which includes the refraction test (determining the eyeglass prescription) and an evaluation of the eye health, which includes checking for glaucoma, cataracts, macular degeneration, etc.

Prior to your examination, there is no way to know if you require a vision or medical eye examination. Towards the beginning of your eye exam, Dr. Weaver will certainly speak with you to review any symptoms or visual complaints you may have and determine the type of visit, because what a patient may consider a routine visit may actually be medical in nature.

Vision Insurance and Medical Health Insurance rules state that we must be consistent across the board with all patients, despite the fact that everyone’s co-pay, deductible and insurance situation is different. Our office does not make these rules and they are defined by the Medical Insurance and Vision Insurance carriers themselves.
